I am faintly amused that Canada is not on this PDF list of either de facto or de jur countries where English is spoken.

I asked them about this, and its not a mistake. It depends on the province.

They clarified that any province outside of Quebec is exempt from TOEFL. Quoting their tweet excerpt back to me:
